HealthMedia Care for Your Health is an individually tailored cognitive behavioural therapy (CBT) program designed to help with the management of chronic conditions. The program emulates a health coaching session, but without the coach, to deliver an individually personalised condition-specific management plan. It teaches you the skills required to effectively manage chronic conditions such as high blood pressure, allergies, back pain, diabetes, high cholesterol, asthma, etc. The HealthMedia Care for Your Health program is designed to help you to:
• Accept your role in managing your disease
• Manage medical, personal and emotional issues
• Manage stress, depression, sleep, fatigue and pain
• Improve your doctor and pharmacist-patient relationships
• Improve medication adherence
• Obtain necessary social support
• Encourage healthy lifestyle choices
